During the coronavirus pandemic, Best Care Essential LLC wants its customers to be assured that we are taking proper precautions to prevent the spread of infection by following the proper safety measures as required by state and local officials. 

In addition, as part of the screening process, all new and current clients who wish to do business with Best Care Essential LLC will be asked to complete a COVID- 19 health screening and Covid Liability Waiver Form.

What is a COVID-19 Health Screening Form?

This COVID-19 Health Screening Form is a document that customers can fill out upon entering into an agreement for business' premises to help protect the health and safety of the community. The form, which acts as a declaration of health, contains questions regarding common symptoms of COVID-19 and asks if the individual or their immediate family members have come in contact with a confirmed case of COVID-19. 

What is a COVID-19 Liability Waiver?

This COVID-19 Liability Waiver is meant to outline the assumption of inherent risk associated with direct patient care or contact. Best Care Essential LLC is a business that involves close contact with people and offer services that are not social-distancing compliant. Therefore, a waiver is meant to protect Best Care Essential of common liability. Gross negligence, such as failure to take proper safety precautions regarding COVID-19 infections, is not protected by this waiver. The enforceability of this waiver in general is a matter of state law.
