Premier Care Package

Transport from the hospital or pick- up location, focus on overall safety and comfort for the patient
Includes:
- 5 hours of nursing care (x 2 days)
- Transport from discharge location to preferred environment or meet you there
- Medication pickup & Review
- Discharge summary review
- monitor vital signs/ongoing nursing assessments

Premium Care Package
$660
Premier care plus pain management, light meal prep, and a focus on rest and relaxation for the patient
Includes:
- 7 hours of nursing care (x 3 days)
- Pain management
- Light meal prep (client provided)
- Rest and relaxation techniques/support
Deluxe Care Package
$960
Premier and Premium care plus care team consultation at discharge, medication organization for the week, environment readiness, education for helpers (family/friends), and care management monitoring
Includes:
- 10 hours of nursing care (x 4 days)
- Meet with you and your care team at discharge
- Medication organization for the week
- Environment readiness
- Education for helpers (includes family/friends)
- Care management/monitoring
